People diagnosed with a bipolar disorder often have issues with alcohol and substance use. In fact, many people with bipolar disorder meet the clinical for an alcohol and/or substance use disorder (AUD/SUD). When an individual has a mental health disorder such as bipolar disorder and an alcohol and/or substance use disorder at the same time, they receive a dual diagnosis and have what mental health treatment providers call co-occurring disorders.

Data from the National Institute of Mental Health (NIMH) shows the following prevalence rates for bipolar disorder (BD) in the U.S.:

  • 2.8% of adults report a diagnosis of BD in the past year
  • 4.4% of adults report a diagnosis of BD in their lifetime
  • 82.9% of adults with BD report BD with severe impairment
  • 17.1% of adults with BD report BD with moderate impairment

  • AUD among people with BD increases suicide risk

One explanation for the high rates of AUD and SUD among people with bipolar disorder is the phenomenon of self-medication: a person with bipolar disorder may seek to control the manic episodes related to bipolar disorder with alcohol or drugs, or they may seek to manage the discomfort of the depressive episodes related to bipolar disorder with alcohol or drugs. In either case, self-medication offers only temporarily relief, and causes more problems than it solves.
